2. Tell the customer service executive you want to remove your online account and provide the asking information.

Remove Your Account by Sending Mail

1. Open your email account that is registered with account.
2. Now compose a email and enter the email address Privacy@cms.hhs.gov
3. On Subject Type “REQUEST TO REMOVE MY ACCOUNT”.
4. Now write an email regarding deleting your account
